- আবেদনের সঙ্গে কাগজপত্র সরকারি বিদ্যালয়ের প্রধান শিক্ষক বা ইউনিয়ন পরিষদের সদস্য/নারী সদস্যদের স্বাক্ষরসহ সিল বাধ্যতামূলক।
- যাদের জন্ম ২০০১ সালের ১ জানুয়ারির পর তাদের পিতা-মাতা মৃত হলে প্রথমে অনলাইন জন্ম নিবন্ধন গ্রহণ করার পর অনলাইন মৃত্যু নিবন্ধন সনদ গ্রহণ করতে হবে। উভয় সনদ আবেদনপত্রের সঙ্গে জমা দিতে হবে।
